Gentamicin Injection

Description:
Optimycin Injection contains Gentamicin
Sulphate, which is an aminoglycoside antibiotic. Gentamicin
has proven bactericidal action against an extremely broad spectrum
of gram-positive and gram–negative pathogens including E.
coli, Klebsiella, Proteus, Pseudomonas aeruginosa
and penicillin resistant strains of Staph. aureus. The
exact mechanism of action is unknown but in common with other
aminoglycoside antibiotics it appears to interfere with protein
synthesis of bacterial cell.
Composition:
Optimycin 20 Injection: Each 2 ml ampoule
contains Gentamicin Sulphate USP equivalent to Gentamicin 20
mg
Optimycin 80 Injection: Each 2 ml ampoule
contains Gentamicin Sulphate USP equivalent to Gentamicin 80
mg
Indications:
Optimycin Injection is indicated in septicemia
and neonatal sepsis, meningitis and other CNS infections; chest
infections; biliary tract infections; acute pyelonephritis or
prostatitis; endocarditis caused by Streptococci viridans or
faecalis (with a penicillin); wound infection & burns, gonorrhea
and persistent stains of both Gram-negative & Gram-positive
organisms. It is also indicated in pre -& post-operative
infections, bone & soft tissue infections.
Dosage and administration:
Optimycin Injection is given by intramuscularly
or by intravenous injection over at least 3 minutes or by intravenous
infusion.
Adult:
2-5 mg/Kg daily in divided doses every 8 hours,
if renal function is normal. Patients with impaired renal function
the interval between doses should be increased to 12 hours when
the creatinine clearance is 30-70 ml/min, 24 hours for 10-30
ml/min, 48 hours for 5-10 ml/min or may be given as directed
by the physician.
Streptococcal or enterococcal endocarditis in
combination with other drugs, 80 mg twice daily.
Child:
Up to 2 weeks: 3 mg / Kg every 12 hours, 2 weeks-12
years: 2 mg / Kg every 8 hours.
Contraindications:
Optimycin Injection is contraindicated
to patients hypersensitive to Gentamicin. It is also contraindicated
in myasthenia gravis.
Side-effects:
Vestibular and auditory damage, nephrotoxicity;
rarely, hypomagnesaemia on prolonged therapy, antibiotic associated
colitis; also reported, nausea, vomiting, rash etc.
Precautions & Warnings:
Renal impairment, early pregnancy, premature
infants & elderly, myasthenia gravis, Parkinsonism.
Use in Pregnancy:
Where possible aminoglycosides should be avoided
in pregnancy as they cross the placenta and can cause fetal
eighth nerve damage.
Drug Interactions:
Potent diuretics. Increases toxicity with nephrotoxic
agents. Prolongs effects of neuromuscular blockers.
Admixture Incompatibility:
Do not physically premix Gentamicin with other
drugs; administer separately in accordance with route and dosage
schedule.
Storage:

Store between 2-30º C temperature.
